Hospice RN Case Manager

Responsibilities

• Manage a patient caseload including assessing, planning, implementing, and evaluating nursing processes
• Coordinate care with attending physicians, hospice staff, and other agencies
• Provide accurate and timely documentation in electronic medical records
• Coordinate physical care by teaching caregivers and providing direct care
• Participate in Interdisciplinary Group meetings and patient care conferences
• Provide support to patients and families during time of death and bereavement
• Supervise and document NAC every two weeks

Requirements

• Current license to practice as a Registered Nurse in the State of Virginia
• At least two years of professional nursing experience
• Previous experience in hospice care preferred
• Valid driver's license and reliable vehicle for year-round travel
• Ability to work independently and as part of a multidisciplinary team
