About the role
Responsibilities
- Manage a patient caseload including assessing, planning, implementing, and evaluating nursing processes
- Coordinate care with attending physicians, hospice staff, and other agencies
- Provide accurate and timely documentation in electronic medical records
- Coordinate physical care by teaching caregivers and providing direct care
- Participate in Interdisciplinary Group meetings and patient care conferences
- Provide support to patients and families during time of death and bereavement
- Supervise and document NAC every two weeks
Requirements
- Current license to practice as a Registered Nurse in the State of Virginia
- At least two years of professional nursing experience
- Previous experience in hospice care preferred
- Valid driver's license and reliable vehicle for year-round travel
- Ability to work independently and as part of a multidisciplinary team
