Hospice RN Case Manager

About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Our clinicians go to the home of the patient, providing a continuum of care for those with chronic conditions and limited mobility through various programs including remote patient monitoring and behavioral health management.

Responsibilities

• Manage assigned patient caseload within the territory
• Assess, plan, implement, and evaluate patient care throughout the nursing process
• Document all notes in the EMR accurately and in a timely manner
• Communicate and partner cross-departmentally with physicians and hospice staff
• Educate and coach relevant parties on best practices regarding levels of care
• Attend patient care conferences and facilitate discussions
• Supervise and document the NAC every two weeks
• Provide support at the time of death and during bereavement for families

Requirements

• Valid State license to practice as a Registered Nurse in Pennsylvania
• 1-2+ years of professional nursing experience, preferably in hospice care
• Ability to assess and respond to patient and caregiver needs in varied settings
• Skilled in nursing practice and able to manage emotional stress
• Valid driver’s license and a reliable vehicle for year-round travel
• Ability to work independently and as part of a multidisciplinary team
