Hospice RN Case Manager

Responsibilities

• Manage assigned patient caseload within the territory
• Assess, plan, implement, and evaluate patient care
• Document all notes in the EMR accurately and timely
• Partner cross-departmentally with physicians and hospice staff
• Educate and coach parties on best practices for care levels
• Attend patient care conferences and facilitate discussions
• Supervise and document the NAC every two weeks
• Provide support at time of death and during bereavement
• Maintain familiarity with agency policies and state/federal regulations

Requirements

• Valid Pennsylvania Registered Nurse (RN) license
• 1-2+ years of professional nursing experience
• Previous hospice care experience preferred
• Ability to assess and respond to patient needs in varied settings
• Valid driver’s license and reliable vehicle for year-round travel
• Strong communication and documentation skills
• Ability to work independently and as part of a multidisciplinary team
