About the role
Responsibilities
- Manage assigned patient caseload within the territory
- Assess, plan, implement, and evaluate patient care
- Document all notes in the EMR accurately and timely
- Partner cross-departmentally with physicians and hospice staff
- Educate and coach parties on best practices for care levels
- Attend patient care conferences and facilitate discussions
- Supervise and document the NAC every two weeks
- Provide support at time of death and during bereavement
- Maintain familiarity with agency policies and state/federal regulations
Requirements
- Valid Pennsylvania Registered Nurse (RN) license
- 1-2+ years of professional nursing experience
- Previous hospice care experience preferred
- Ability to assess and respond to patient needs in varied settings
- Valid driver’s license and reliable vehicle for year-round travel
- Strong communication and documentation skills
- Ability to work independently and as part of a multidisciplinary team
