Hospice RN Case Manager

For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care.

Responsibilities

• Assist in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families.

Requirements

• Current NYS Registered Nurse license
• Two years of nursing experience (medical-surgical, oncological, palliative, or community home health/hospice)
• Ability to commute to multiple territories to ensure continuity of care
• Valid driver’s license (if not assigned exclusively to Manhattan)
• Vehicle required for patient commutes (if not assigned exclusively to Manhattan)

Preferred Qualifications

• One year of community hospice or home health experience
• Hospice experience
• Baccalaureate Degree in Nursing from an accredited school
