About the role
Responsibilities
- Provide exceptional end-of-life care for patients and their families in home settings
- Assess patient needs and develop individualized care plans
- Implement care plans to ensure the highest level of comfort and quality of life
- Support patients and families through the end-of-life journey with compassion
Requirements
- Registered Nurse (RN) license
- Minimum of one year of experience in an acute care setting or comparable work experience
Preferred Qualifications
- Minimum of one year of oncology experience
- Minimum of one year of home care experience
- Minimum of one year of Hospice experience
