Responsibilities
- Provide skilled end-of-life care to patients and families
- Serve as a mentor, educator, and clinical resource for nurses
- Manage a patient caseload and coordinate individualized care
- Guide new clinicians through orientation and provide coaching
- Promote best practices and reinforce quality standards
Requirements
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum 2 years of experience in hospice or home health
- Reliable transportation, valid driver’s license, and automobile insurance

- BSN preferred
