Hospice Registered Nurse

Responsibilities

• Deliver skilled, compassionate nursing care directly in patients' homes.
• Complete initial and ongoing home assessments, including full physical evaluations and medical history reviews.
• Perform accurate OASIS assessments and ensure documentation reflects patient status and care needs.
• Develop individualized care plans with measurable goals, involving patients and families in all planning.
• Coordinate interdisciplinary care by communicating significant changes to physicians and obtaining necessary orders.
• Teach and counsel patients and families on self-care techniques, disease management, and safety.
• Prepare timely, accurate clinical notes and maintain strong communication with the interdisciplinary team.
• Travel within an assigned territory to provide care and support quality outcomes.

Requirements

• Associate Degree in Nursing (BSN preferred).
• Active New Hampshire Board of Nursing license.
• At least one year of recent acute care experience (experience in hospice, palliative, or oncology care is a plus).
• BLS/CPR certification.
• Valid Driver's license and ability to meet driving and insurance requirements.
• Proficiency with EMR systems (Allscripts preferred) and knowledge of OASIS.
