Hospice Registered Nurse - Case Manager

Responsibilities

• Complete initial and ongoing home assessments, including full physical evaluations and medical history reviews.
• Perform accurate OASIS assessments and ensure documentation reflects patient status and care needs.
• Develop individualized care plans with measurable goals, involving patients and families in all planning and revisions.
• Coordinate interdisciplinary care by communicating significant changes to physicians and obtaining necessary orders.
• Teach and counsel patients and families on self-care techniques, disease management, and safety.
• Prepare timely, accurate clinical notes and maintain strong communication with the interdisciplinary team.
• Travel within assigned territory to provide skilled nursing care directly in the patient's home.

Requirements

• Associate Degree in Nursing (BSN preferred).
• New Hampshire Registered Nurse license in good standing.
• At least 1 year of recent acute care experience (hospice, palliative, or oncology experience preferred).
• BLS/CPR certification.
• Valid Driver's License.
• Ability to meet driving, insurance, and background requirements.

Preferred Qualifications

• Experience with OASIS and home health reimbursement.
• Proficiency with EMR systems, specifically Allscripts.
