Hospice Registered Nurse Case Manager

About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Our clinicians go to the home of the patient, providing a continuum of care for those with chronic conditions and limited mobility through various programs including remote patient monitoring and behavioral health management.

Responsibilities

• Assume primary responsibility for a patient caseload including assessing, planning, implementing, and evaluating nursing processes
• Initiate communication with attending physicians and hospice staff to coordinate optimal care
• Provide accurate and timely documentation in electronic medical records
• Coordinate physical care by teaching caregivers or providing direct care
• Participate in Interdisciplinary Group meetings and patient care conferences
• Provide support at the time of death and during bereavement periods
• Maintain familiarity with agency policies and state/federal hospice regulations

Requirements

• Current license to practice as a registered nurse in the State of VA
• At least two years of professional nursing experience
• Previous experience in hospice care preferred
• Valid driver’s license and reliable vehicle for year-round travel
• Ability to work independently and as part of a multidisciplinary team
• Strong assessment and clinical documentation skills
