Hospice Registered Nurse Case Manager

Responsibilities

• Manage a patient caseload including assessing, planning, implementing, and evaluating nursing processes
• Coordinate care with attending physicians, hospice staff, and other agencies
• Provide accurate and timely documentation in electronic medical records
• Conduct Goals of Care discussions with patients
• Participate in discharge planning for GIP and RHC discharges into the community
• Perform hospice evaluations and provide daily GIP level of care visits
• Support routine home visits and admissions in the field as needed
• Provide support during time of death and bereavement periods

Requirements

• Current license to practice as a Registered Nurse in the State of Georgia
• 1-2 years of professional nursing experience, preferably in hospice care
• Valid driver's license and reliable vehicle for year-round travel
• Ability to work independently and as part of a multidisciplinary team
• Skilled in making meaningful observations and writing comprehensive reports
