Hospice Registered Nurse Case Manager

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Our clinicians provide a continuum of care for those with chronic conditions and limited mobility, ensuring patients receive high-quality care through programs like remote patient monitoring and behavioral health management.

Responsibilities

• Manage a patient caseload including assessing, planning, implementing, and evaluating nursing processes
• Coordinate care with attending physicians, hospice staff, and other agencies
• Provide accurate and timely documentation in electronic medical records
• Participate in Interdisciplinary Group meetings and patient care conferences
• Coordinate physical care by teaching caregivers or providing direct care
• Supervise and document the NAC every two weeks
• Provide support during the time of death and bereavement periods
• Maintain familiarity with agency policies and state/federal hospice regulations

Requirements

• Graduate from an accredited School of Nursing
• Licensed Registered Nurse in New Jersey
• One year of RN experience
• Fluent in both English and Spanish
• Licensed driver with an insured automobile in good working order
• Excellent assessment and problem-solving skills

Preferred Qualifications

• Previous hospice experience
