Hospice Quality RN

Responsibilities

• Elicit recommendations for continuous improvement of quality and patient safety programs
• Investigate occurrences and lead root cause analyses
• Lead implementation and monitoring of quality, patient safety, and performance improvement programs
• Develop and lead Performance Improvement education programs for all staff
• Support departments in identifying annual performance improvement initiatives
• Oversee data collection, chart abstraction, and reporting activities
• Manage Quality Assurance and Performance Improvement (QAPI) reports and committee meeting preparation
• Utilize system-wide dashboards to track Quality & Safety activities
• Act as a liaison to clinical departments to assess problems and monitor actions

Requirements

• Valid Registered Nurse (RN) license in good standing
• Proven experience in hospice, palliative medicine, critical care, ICU, emergency, or medical-surgical units
• Minimum of 5 years of nursing experience
• Minimum of 2 years of experience in Quality Assessment, Patient Safety, or Performance Improvement
• Proficiency with data, analysis, and reporting
• Experience with EMR/EHR systems
• Strong communication skills for interdisciplinary collaboration and family education
• Knowledge of infection control practices and aseptic techniques
