Hospice Nurse Practitioner

Responsibilities

• Provide palliative care consultation to patients within community settings
• Perform physical assessments for patients facing serious and complex illnesses
• Work within a collaborative team practice that maintains the patient as the central focus
• Communicate effectively with patients and families regarding care plans

Requirements

• Master’s of Science in Nursing (MSN)
• Nurse Practitioner Certification in area of specialty
• Licensed to practice in Massachusetts
• Current DEA registration and Mass DPH Controlled Substances Registration
• Minimum of 2 years of nursing experience
• Excellent computer skills and ability to multitask in a rapidly changing environment

Preferred Qualifications

• Experience with electronic medical records (EMR) systems
