About the role

Responsibilities

  • Provide consultation in palliative care, symptom management, and supportive care to meet the needs of hospice patients.
  • Conduct face-to-face encounter visits within prescribed time limits and report clinical findings to the Hospice Medical Director.
  • Consult with the Hospice Medical Director and collaborate with primary physicians and other healthcare providers as needed.
  • Initiate and coordinate the plan of care, documenting problems, goals, interventions, and patient/family responses.
  • Instruct and supervise patients and their families in self-care techniques when appropriate.
  • Maintain accurate and relevant clinical notes regarding the patient's condition and report issues to the Administrator and Medical Director.

Requirements

  • Active unrestricted license in the state of residence.
  • Current certification as a Clinical Nurse Specialist or Nurse Practitioner in the state of practice.
  • National certification from a body such as ANCC, NBCHPN, or AANP.
  • 5+ years of nursing experience.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Preferred Qualifications

  • Current CPR certification (or ability to complete within 90 days of hire).
  • Experience working within an interdisciplinary team.
  • Clinical nursing experience in oncology, hospice, geriatrics, or intensive care.
  • Palliative Care ARNP certification.
