Hospice Medical Social Worker

Responsibilities

• Provide compassionate, clinical psychosocial care to patients and families navigating end-of-life illness
• Complete comprehensive psychosocial assessments
• Provide crisis intervention and counseling
• Support advance care planning conversations
• Collaborate with nurses, physicians, and other hospice professionals within an interdisciplinary team
• Integrate emotional, cultural, and practical needs into patient plans of care

Requirements

• Master's or doctoral degree from a CSWE-accredited school of social work
• Current State License as a Master's Social Worker (MSW/LMSW/LCSW)
• Minimum 1 year of experience as a licensed Master's Social Worker
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage
• Willingness to travel to homes, facilities, and community settings

Preferred Qualifications

• Hospice or palliative care experience
• Clinical experience with grief counseling and end-of-life issues
• Knowledge of Medicare hospice benefit and Conditions of Participation
