Hospice Medical Social Worker

For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care.

Responsibilities

• Provide medical social services to hospice patients and families
• Deliver psychosocial and bereavement counseling
• Commute to multiple territories to ensure continuity of care

Requirements

• MSW from an accredited school of social work
• One year of related experience
• New York LMSW or LCSW eligibility
• Valid driver’s license
• Access to a vehicle for patient commutes (except for Manhattan territory)
• Excellent interpersonal and communication skills

Preferred Qualifications

• Minimum one year of bereavement counseling experience
• At least one year of Hospice or Home Care experience
