Hospice Intake Nurse

Responsibilities

• Answer phones and receive all referral and intake information
• Complete referral/intake forms and route them for admission approval
• Assist in scheduling appropriately skilled personnel to fill cases
• Input scheduling information into the computer system
• Check common working files for Medicare Coverage and verification of benefit periods
• Perform other duties as directed

Requirements

• High school graduate or GED
• Multi-state LPN license
• 3 years of customer service experience
• 3 years of EMR system experience
• Computer literacy and ability to maintain simple records
• Effective oral and written communication skills

Preferred Qualifications

• Experience in a Home Health Care or Hospice setting
