About the role
Responsibilities
- Assist patients with personal care activities including bathing, grooming, and dressing
- Maintain a clean and safe environment for patients in their homes
- Provide emotional support to patients and their families during the end-of-life journey
- Effectively communicate with healthcare team members and family caregivers
- Document care provided and patient progress accurately and in a timely manner
- Maintain strict confidentiality of patient, family, and caregiver information
Requirements
- High school diploma or equivalent
- Valid Massachusetts CNA or HHA certification
- Valid Driver's license and access to a personal vehicle
- Strong communication and interpersonal abilities
