Hospice Chaplain

Responsibilities

• Provide direct spiritual care and assessments to patients, families, and caregivers within the hospice program
• Facilitate the development of Interdisciplinary Team Plans of Care to address identified spiritual needs
• Serve as a liaison and support to community chaplains and spiritual counselors
• Maintain accurate records of spiritual care services for quality assurance and program development
• Participate in Interdisciplinary Team meetings and provide consultation on spiritual care matters
• Recruit and coordinate community chaplains, spiritual counselors, and volunteers
• Assist with or plan funeral, memorial, or periodic services for patients and families

Requirements

• Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree)
• Successful completion of at least one accredited Clinical Pastoral Education (CPE) program
• Experience working with individuals, families, and caregivers facing death and dying
• Member in good standing with your religious denomination
• Ability to work effectively within an interdisciplinary team in an interfaith setting
• Comfort working with diverse lifestyles, cultures, beliefs, and values
• Must be based in the greater Chicagoland area and able to travel in the field between patient homes

Preferred Qualifications

• Previous experience working in a hospice setting
