Theological degree (BD, MDiv or equivalent), Experience with end-of-life care, Licensed driver with insured vehicle, Interfaith competency

About the role

Responsibilities

  • Assure spiritual assessment of patients and families/caregivers in the hospice program
  • Facilitate the development of individualized plans of care to meet spiritual needs
  • Provide direct spiritual care to patients and families/caregivers
  • Serve as liaison and support to community chaplains and spiritual counselors
  • Maintain records of spiritual care services utilization and related activities
  • Document direct services and ongoing communication with community chaplains
  • Attend patient care conferences and interdisciplinary group meetings
  • Provide consultation, education, and support to the interdisciplinary group
  • Recruit community chaplains, spiritual counselors, and volunteers
  • Provide for funeral or memorial services as requested
  • Plan periodic memorial services for personnel and community clergy
  • Assist in supervision of spiritual care volunteers
  • Participate in quality assessment and performance improvement activities

Requirements

  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent)
  • Experience working with death, dying, and individuals/families/caregivers
  • Ability to work as a member of an interdisciplinary group
  • Comfort in an interfaith setting
  • Ability to accept different lifestyles, cultures, beliefs, and values
  • Ability to network with community clergy and congregations
  • Knowledge of and commitment to hospice philosophy of care
  • Licensed driver with a reliable, insured automobile in good working order

Preferred Qualifications

  • Previous hospice experience
