Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care.

Responsibilities

• Develop relationships and maintain regular contact with referral sources including physicians, hospitals, and nursing facilities
• Determine patient eligibility for hospice care
• Admit patients and obtain informed consents for service
• Inform patients and families of their rights and responsibilities
• Assist with the implementation and coordination of services for patient discharge from hospitals into the hospice program
• Communicate with Intake and Admissions departments regarding referrals
• Assist the Director of Provider Relations in negotiating or redefining contract provisions

Requirements

• AA or B.S. degree in Nursing from an accredited Nursing School
• Minimum of one year of experience in a home health or hospice setting
• Current license as a Registered Professional Nurse in the State of California

Preferred Qualifications

• Public Health Nursing certification
