Responsibilities

• Perform personal care needs and attend to patient requests promptly
• Provide positive communication to patients and families while maintaining confidentiality
• Use medical equipment safely and properly
• Provide personal care including baths, oral hygiene, and changing bed linen
• Assist with dressing, undressing, and feeding
• Plan and prepare nutritious meals as assigned
• Take and record vital signs including temperature, pulse, respiration, and blood pressure
• Observe and report changes in patient condition to the Case Manager
• Assist in ambulation, exercise, and range of motion exercises
• Assist with self-administered medication reminders
• Maintain a clean and orderly living area for the patient

Requirements

• Active STNA or CNA certification in the state of performing duties
• Completion of minimum 75 hours of classroom and supervised practical training
• At least 18 years of age
• Ability to read and follow written instructions and document care
• Current CPR certification
• Licensed driver with a reliable, insured automobile
• Ability to lift and/or move up to 50 pounds
