Responsibilities

• Perform personal care needs including baths, back rubs, oral hygiene, and shampoos
• Assist with dressing, undressing, feeding, and ambulation
• Observe and report patient condition and significant changes to the Case Manager
• Take and record vital signs such as temperature, pulse, respiration, and blood pressure
• Plan and prepare nutritious meals and assist with feeding
• Maintain a clean and orderly living area for the patient
• Provide assistance with self-administered medications as allowed by state regulations
• Assist with transfers, positioning, and range of motion exercises
• Adhere to documentation and care procedures and professional conduct standards

Requirements

• Active STNA or CNA certification in the state of practice
• Completion of minimum 75 hours of classroom and supervised practical training
• At least 18 years of age
• Current CPR certification
• Licensed driver with a reliable, insured automobile in good working order
• Ability to lift and/or move up to 50 pounds
• Ability to read and follow written instructions and document care

Preferred Qualifications

• Experience in hospice or end-of-life care
• Demonstrated empathy and patience for the ill and impaired
• Ability to work with little direct supervision
