Hospice Admissions Registered Nurse (RN)

Responsibilities

• Provide specialized in-home nursing services in accordance with the established plan of care
• Manage a caseload of approximately 12-18 patients, adjusting for complexity and geography
• Demonstrate competency in pain management, symptom control, infusion therapy, and wound care
• Address the physical, emotional, psychosocial, and spiritual needs of patients and their families
• Coordinate the total Plan of Care by liaising with other health professionals and interdisciplinary teams
• Evaluate and re-evaluate nursing needs routinely, making necessary revisions to care plans
• Educate patients and caregivers on their rights and responsibilities as hospice patients
• Communicate changes in patient condition to Attending Physicians and the Hospice Medical Director
• Act as a Case Manager from admission through discharge, including coordination of bereavement services

Requirements

• Graduate of an accredited nursing program
• Active RN license in the state of practice
• Minimum of 1 year of hospice experience within the last 24 months
• Valid driver's license, reliable transportation, and auto insurance
• Current CPR certification
• Ability to pass background and sex-offender checks
