About the role
Responsibilities
- Facilitate the admission process for new patients to ensure a smooth transition into hospice care
- Conduct thorough clinical assessments to gather medical history and determine hospice eligibility
- Collaborate with physicians and interdisciplinary teams to develop individualized care plans
- Educate patients, families, and caregivers regarding hospice benefits and services
- Coordinate with social workers, chaplains, and volunteers for necessary admission services
- Administer medications and treatments following established protocols for pain and symptom management
- Monitor vital signs and patient condition, reporting changes to the healthcare team
Requirements
- Graduate of an accredited school of Nursing
- Current State License as a Registered Nurse
- Minimum 1 year of experience as a nurse in a clinical care setting
- Strong clinical skills and communication abilities
