Hospice Admission Registered Nurse

Responsibilities

• Facilitate the admission process for new patients to ensure a smooth transition into hospice care
• Conduct thorough clinical assessments to gather medical history and determine hospice eligibility
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans
• Coordinate with social workers, chaplains, and volunteers for necessary services
• Administer medications and treatments following established protocols for pain and symptom management
• Monitor patient vital signs and report changes to the healthcare team
• Complete all admission paperwork, including consent forms and medical records, accurately

Requirements

• Graduate of an accredited school of Nursing
• Current State License as a Registered Nurse
• Minimum 1 year of experience as a nurse in a clinical care setting
• Strong clinical skills and excellent communication abilities
• Ability to handle multiple priorities and documentation deadlines
