Homelessness Prevention Case Manager

Responsibilities

• Conduct prescreens and intakes to determine participant eligibility and assess assistance needs
• Manage a caseload of up to 15 or more households, developing strength-based service plans to ensure housing stability
• Provide families with information, referrals, and connections to community resources and public benefits
• Act as an advocate through landlord mediation, tenant rights education, and connections to legal services
• Support families with budget counseling, financial literacy, and immediate crisis intervention
• Document all program activity, including intakes and financial assistance, in HMIS and Salesforce databases
• Participate in community outreach and build collaborative relationships with partners like schools and the court system

Requirements

• Bachelor’s degree in social work, public health, or a related field (or equivalent experience)
• Three years of experience in human services or a related field
• Bilingual and bicultural proficiency in English and Spanish (written and verbal)
• Proficiency in MS Office (Word, Excel, PowerPoint) and Google Suite
• Valid CA driver’s license, reliable personal vehicle, and vehicle insurance
• Proof of full COVID-19 vaccination
• Ability to pass a background check

Preferred Qualifications

• Two years of experience working in outreach, shelter, or supportive housing programs
• Experience working with landlords, local rental markets, or Housing First focused non-profits
