Homecare Pharmacist

Responsibilities

• Act as the Responsible Pharmacist to deliver prescription management and dispensing in line with operational KPIs and safety standards
• Conduct clinical screening and interventions for prescription receipts according to standard operating procedures
• Accurately dispense prescriptions and ensure timely reporting of near misses and dispensing errors
• Manage product orders and authorize prescription entries on the Patient Medication Record
• Formulate unlicensed medicines and coordinate with manufacturing providers within contractual timeframes
• Collaborate with clinical teams to ensure patient adherence and escalate issues as necessary
• Record and escalate adverse events and incidents to the appropriate quality and safeguarding leads
• Provide clinical counseling and product expertise to patients, care teams, and pharmacy support staff
• Mentor and coach pharmacy support staff through dedicated training and development

Requirements

• Registered Pharmacist with the GPhC
• Proven experience in conducting clinical investigations, such as near-miss investigations
• Strong coaching and training capabilities with the ability to motivate others
• Excellent problem-solving, analytical, and critical thinking skills
• Commitment to continuous professional development (CPD) and maintaining up-to-date knowledge of GPhC and RPS standards

Preferred Qualifications

• Post-registration experience working specifically within a homecare pharmacy setting
