Home Visit Nurse (RN)

Responsibilities

• Conduct postpartum home visits to provide physical assessments for mothers and newborns
• Assess family needs across 12 domains of physical and psychosocial well-being
• Facilitate referrals and connections to community resources and agencies
• Follow the evidence-based Family Connects home-visiting and scheduling protocols
• Document all client observations, assessments, and activities in real time via computerized systems
• Teach at least one in-person prenatal education class per week, covering topics like newborn care and breastfeeding
• Collaborate with clinical and non-clinical team members to support family health plans

Requirements

• B.S. in Nursing with current RN license, or B.A. in another field with current RN license, or A.S. in Nursing with at least one year of relevant nursing experience
• Current Illinois RN Licensure
• Current CPR certification through the American Heart Association
• Valid driver’s license and an automobile for home visits
• Ability to lift at least 50 pounds
• Ability to work with culturally diverse populations and on weekends as required

Preferred Qualifications

• Spanish language proficiency
• Clinical experience related to domestic violence, substance abuse, or depression
• One or more years of experience in maternal-child health or social work
• Certified Lactation Educator or Consultant
