Home Therapy LPN

Responsibilities

• Provide safe and effective dialysis therapy for patients in a home therapy setting under the supervision of a Registered Nurse
• Administer intravenous medications in accordance with physician orders and state nurse practice laws
• Perform ongoing monitoring, assessment, and intervention for home patients, including home visits and clinical appointments
• Assist in the identification, evaluation, and training of candidates for the home dialysis program
• Maintain accurate documentation of patient training and treatment in the Medical Information System
• Troubleshoot equipment problems with patients via telephone and ensure appropriate resolution
• Manage patient supply inventory and ensure maintenance of durable medical equipment
• Collaborate with physicians and the Home Therapy RN/Coordinator to adjust patient treatment plans as needed

Requirements

• Graduate of an accredited School of Practical Nursing (LPN/LVN)
• Current and appropriate state licensure
• Valid CPR certification
• Strong verbal and written communication skills
• Ability to travel between assigned facilities and various community locations

Preferred Qualifications

• Experience in medical-surgical nursing
• Experience in hemodialysis
• Experience in ICU settings
