Responsibilities

• Provide direct patient care and administer intravenous medication for assigned patients in their homes under the supervision of a Registered Nurse.
• Monitor, assess, and intervene for home patients, including conducting home visits, lab studies, and annual reviews.
• Accurately document patient training, treatments, and medical information in the Medical Information System.
• Assist in identifying, evaluating, and training candidates for the home dialysis program.
• Manage patient supplies, inventory, and ensure the maintenance of durable medical equipment.
• Maintain technical knowledge of dialysis equipment and troubleshoot problems via telephone communication.
• Participate in staff meetings, patient care conferences, and educational in-service classes.

Requirements

• Graduate of an accredited School of Practical Nursing (LPN/LVN).
• Current and appropriate state licensure.
• Minimum of 2 years of Hemodialysis experience.
• Successful completion of a hemodialysis training course in theory and practice.
• Current CPR certification.
• Ability to work independently and manage emergencies.
• Strong verbal and written communication skills.

Preferred Qualifications

• One year of medical-surgical nursing experience.
