Home Health Registered Nurse (RN)

Responsibilities

• Assess new patient condition, history, and home care needs
• Build and update care plans with patients and families
• Administer medications and treatments as prescribed
• Teach patients and families how to manage care at home
• Coordinate with physicians and community health partners
• Plan and carry out discharge processes
• Arrange necessary equipment or services
• Monitor home health aide care every two weeks

Requirements

• Graduate of an accredited school of nursing
• 1-2 years of recent acute care experience in an institutional setting
• Current state nursing licensure
• CPR certification
• Licensed driver with a reliable, insured automobile
• Ability to lift and carry up to 50 lbs occasionally
• Ability to perform nursing duties with sufficient visual and hearing acuity

Preferred Qualifications

• Bachelor's degree
• 1 year of home health care experience
