Home Health Physical Therapist

Responsibilities

• Provide one-on-one rehabilitation care in patients' homes, assisted living facilities, or nursing homes
• Assess patients' functional abilities and home environments
• Develop and implement individualized treatment plans
• Guide patients through targeted interventions to improve mobility, balance, strength, and safety
• Educate and empower patients and caregivers
• Collaborate with an interdisciplinary care team to improve patient outcomes

Requirements

• Master's or doctoral degree from an accredited Physical Therapy program
• Current state license as a Physical Therapist
• Minimum 1 year of experience as a licensed Physical Therapist
• Reliable transportation, valid driver’s license, and valid automobile insurance
• Willingness to travel daily within the service area

Preferred Qualifications

• Home health experience
• OASIS certification or willingness to obtain
• Experience with Medicare home health documentation
