Home Health Physical Therapist

Responsibilities

• Assess patient functional status, muscle strength, mobility, gait, and rehab potential
• Execute physical therapy plans of care as set by physicians
• Instruct patients and families on therapeutic appliances and prosthetic devices
• Document clinical care and progress summaries accurately and timely
• Coordinate with physicians regarding patient response to treatment
• Supervise physical therapy assistants according to state regulations
• Participate in discharge planning and case conferences

Requirements

• Degree from an accredited baccalaureate or Master's program in physical therapy
• Licensed to practice as a physical therapist in the state
• Two years of appropriate physical therapy experience
• Current CPR certification
• Valid driver's license and reliable automobile
• Ability to lift and carry up to 50 lbs occasionally

Preferred Qualifications

• Previous experience in community or home health settings
