Home Health Aide

Responsibilities

• Assist with personal care including bathing, dressing, grooming, toileting, and hygiene
• Support safe mobility through ambulation, transfers, and range of motion exercises
• Monitor and report vital signs and changes in patient condition
• Assist with meals, feeding, and light housekeeping
• Support medication self-administration as directed
• Maintain a clean and safe patient environment
• Document care accurately and timely in the EMR
• Communicate proactively with nurses and care team members

Requirements

• Active CNA or Home Health Aide certification in good standing
• Current CPR certification
• Reliable transportation, valid driver’s license, and auto insurance
• Ability to work independently in a field-based setting
• Strong observation and communication skills

Preferred Qualifications

• Previous home health experience
• Confidence working one-on-one with patients in their homes
• Ability to recognize and escalate changes in condition quickly
