Home Health Aide

Responsibilities

• Provide hands-on personal care to assigned patients under the direction of a Registered Nurse
• Execute patient visits according to a weekly schedule and established care plans
• Document all visits, including visit reports, routes, and timesheets, in an accurate and timely manner
• Communicate changes in patient condition, abnormal vital signs, or living conditions to the RN Team Leader
• Coordinate with the RN Team Leader to ensure timely completion of assignments and documentation
• Assist patients with medications that are ordinarily self-administered
• Participate in team conferences to provide input regarding patient care and progress toward goals
• Maintain professional standards, confidentiality, and adherence to all organizational policies

Requirements

• Tennessee CNA certification or completion of the required curriculum/hours in accordance with Federal/State regulations
• Valid driver’s license
• Ability to provide hands-on personal care to patients
• Strong documentation and communication skills

Preferred Qualifications

• Prior experience providing home health aide visits
• Working knowledge of home health care programs
