Home Health Aide (HHA)

Responsibilities

• Provide direct personal care including hygiene, bathing, hair care, and oral care
• Assist patients with transfers to or from beds or chairs and perform passive exercises
• Maintain a healthful and clean environment for the patient
• Report significant changes in patient condition or welfare to the CHHA Coordinator
• Complete clinical notes for every visit and submit them in a timely manner
• Perform simple procedures as an extension of therapy services and assist with self-administered medications
• Participate in QA activities, case conferences, and staff meetings

Requirements

• High school diploma or GED with the ability to read and write
• Must pass the nurse aide skills assessment with a score of 75% or higher
• Must demonstrate competency in personal care tasks per the RN agency checklist
• Must complete OBRA competency requirements
• Capable of heavy lifting, excessive standing, and bending
• Must have dependable transportation for daily home visits
• Must be in good health and possess functioning auditory, speech, and visual skills

Preferred Qualifications

• One year of experience working as a Nursing Assistant or Home Health Aide
