Home Care Nurse

Responsibilities

• Provide professional nursing care to patients in home care settings
• Assess patients' physical, psychological, and social needs and develop individualized care plans
• Administer medications, injections, intravenous therapy, and prescribed treatments
• Monitor and record vital signs, patient progress, and response to treatment
• Perform wound care, catheter care, tracheostomy care, PEG feeding, and suctioning
• Assist patients with activities of daily living (ADLs) including hygiene, mobility, and feeding
• Educate patients and family members on disease management and medication compliance
• Maintain accurate nursing documentation and patient records
• Recognize changes in patient conditions and communicate with healthcare teams
• Adhere to infection prevention, control protocols, and patient safety standards
• Ensure compliance with DHA, DOH, or SHA regulations

Requirements

• Bachelor's Degree or Diploma in Nursing
• Valid Registered Nurse (RN) license in home country
• Valid DHA, DOH, or SHA License/Eligibility
• Minimum 2 years of clinical nursing experience
• Current BLS certification
• Strong clinical assessment and patient care skills
• Excellent communication and documentation skills
• Ability to work independently and travel to patients' residences
