Home Care Consultant Nurse

Responsibilities

• Perform pre-installation assessments and initial medical device installations at patient homes (Perfusion, Diabetes, Immunotherapy, etc.).
• Provide therapeutic education to patients and their caregivers regarding equipment use and treatment adherence.
• Coordinate care pathways by collaborating with private nurses (IDEL) and providing technical training when necessary.
• Manage relationships with hospital teams and medical prescribers to ensure strict adherence to therapeutic protocols.
• Maintain fluid communication with internal logistics and technical departments to ensure seamless service delivery.
• Write precise and comprehensive follow-up reports (Comptes-Rendus) for medical prescribers.

Requirements

• State Diploma in Nursing (Diplôme d'État d'Infirmier) is required.
• Significant experience in a hospital setting or home healthcare environment.
• Specialized knowledge in Perfusion and Diabetes management.
• Valid driving license (Permis B) for frequent travel within the Île-de-France region.
• Proficiency in office software and patient tracking/reporting tools.
• Strong organizational skills, autonomy, and pedagogical abilities.
