About the role

Responsibilities

  • Support a high-touch panel of individuals with complex acuity levels through hybrid in-person and virtual support
  • Provide intensive post-discharge management and transitions of care following psychiatric hospitalization
  • Coordinate Substance Use Disorder treatment plans and support medication adherence
  • Develop safety and action plans by understanding individual triggers and supports
  • Co-navigate systemic barriers including appointments, transportation, and care enrollment
  • Resolve social challenges such as housing, food, and transportation
  • Participate in daily huddles and regular interdisciplinary reviews
  • Maintain empathetic, trusting relationships to help individuals avoid traumatic events

Requirements

  • Lived experience with SMI (serious mental illness) and/or SUD (substance use disorder) and recovery
  • Minimum of two years demonstrated recovery time from a mental health or substance abuse disorder
  • State Peer Recovery certification or eligibility to obtain it within six months
  • Ability to complete a paid training period and pass assessments
  • Interest in obtaining a Community Health Worker (CHW) credential
  • Training or willingness to be trained in de-escalation techniques (e.g., CPI, Mental Health First Aid)

Preferred Qualifications

  • Experience with direct in-person engagement for individuals with SMI and SUD
  • Expertise in recovery education and coaching
  • Understanding of trauma-informed approaches to peer support
  • Familiarity with local community resources and behavioral health systems
  • Ability to spend 80%+ of time in the community (homes, hospitals, streets)
  • Valid driver's license in good standing
