Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.

Responsibilities

• Conduct active outreach in the community and correctional facilities to bridge at-risk individuals to treatment
• Deliver group and individual support and HCV treatment and prevention education
• Assist clients with paperwork, medical appointments, and health management planning
• Support the expansion of Naloxone overdose prevention and Harm Reduction programs
• Provide first contact HCV testing and point of care screening
• Facilitate the utilization of peer support workers and provide training on harm reduction
• Design training materials and compile confidential data for reporting
• Participate in program planning, team meetings, and case conferences

Requirements

• Completion of a 2-year post-secondary program in Health or Social Services
• At least one year of experience working in a community setting
• Thorough knowledge of HCV, harm reduction, and peer support models
• Phlebotomy certification or willingness to obtain it
• Strong knowledge of issues affecting marginalized communities
• Excellent interpersonal, presentation, and communication skills
• Ability to work occasional evening and/or weekend shifts
• Valid driver’s license
• Full vaccination against COVID-19
