Health Equity Questionnaire Volunteer

Responsibilities

• Welcome clients upon arrival and guide them to the appropriate person or location
• Assist clients in reading and completing the Health Equity Questionnaire
• Explain questions in a clear and respectful manner
• Follow-up with clients to ensure completion of the questionnaire prior to appointments
• Assist clients in signing in and completing forms on Kiosks
• Record and track form completion
• Maintain confidentiality and follow all privacy policies and guidelines

Requirements

• Strong verbal communication skills
• Comfortable working with individuals from diverse backgrounds
• Ability to respect confidentiality and follow procedures
• Ability to travel to the Oshawa site

Preferred Qualifications

• Medical administration training
