About the role

Responsibilities

  • Coordinate and monitor client physical and behavioral health services within residential treatment facilities
  • Facilitate communication between clients, nursing staff, medical providers, pharmacy services, and program staff
  • Support the intake process and coordinate internal and external medical and behavioral health appointments, including telehealth
  • Assist with medication monitoring and ensure documentation accuracy in the electronic medication administration record (eMAR/MAR)
  • Observe and monitor client changes in physical and behavioral health status
  • Provide direct client support, including health education and reinforcing treatment plans
  • Maintain accurate and timely documentation of services in the electronic health record

Requirements

  • High School Diploma or Equivalent
  • 1 year of experience in caregiving, human services, behavioral health, or social services
  • Ability to apply for Agency Affiliated Counselor registration within 30 days of hire
  • First Aid/CPR certification (must be obtained within 90 days of hire)
  • Basic computer skills and proficiency in written and verbal communication

Preferred Qualifications

  • Experience with electronic health records (EHR) or electronic records management systems
  • Proficiency in Microsoft Office Suite, Outlook, and Internet navigation
  • Certified Nursing Assistant (CNA) certification
