Health Coordinator

Responsibilities

• Coordinate and monitor client physical and behavioral health services within residential treatment facilities
• Facilitate communication between clients, nursing staff, medical providers, pharmacy services, and program staff
• Support the intake process and coordinate internal and external medical and behavioral health appointments, including telehealth
• Assist with medication monitoring and ensure documentation accuracy in the electronic medication administration record (eMAR/MAR)
• Observe and monitor client changes in physical and behavioral health status
• Maintain accurate, timely, and complete documentation of services in the electronic health record
• Provide basic first aid and serve as a first point of contact for emergency response when needed

Requirements

• High School Diploma or Equivalent
• One year of experience in caregiving, human services, behavioral health, or social services
• Ability to apply for Agency Affiliated Counselor registration within 30 days of hire
• Ability to obtain First Aid/CPR certification within 90 days of hire
• Basic computer skills and proficiency in written and verbal communication
• Demonstrated ability to maintain a professional, empathetic, and client-centered approach

Preferred Qualifications

• Experience with electronic health records (EHR) or electronic records management systems
• Proficiency in Microsoft Office Suite, Outlook, and Internet navigation
• Certified Nursing Assistant (CNA) certification
