About the role
Responsibilities
- Coordinate and monitor client physical and behavioral health services within residential treatment facilities
- Facilitate communication between clients, nursing staff, medical providers, pharmacy services, and program staff
- Support the intake process and coordinate internal and external medical and behavioral health appointments, including telehealth
- Assist with medication monitoring and ensure documentation accuracy in the electronic medication administration record (eMAR/MAR)
- Observe and monitor client changes in physical and behavioral health status
- Maintain accurate, timely, and complete documentation of services in the electronic health record
- Provide basic first aid and serve as a first point of contact for emergency response when needed
Requirements
- High School Diploma or Equivalent
- One year of experience in caregiving, human services, behavioral health, or social services
- Ability to apply for Agency Affiliated Counselor registration within 30 days of hire
- Ability to obtain First Aid/CPR certification within 90 days of hire
- Basic computer skills and proficiency in written and verbal communication
- Demonstrated ability to maintain a professional, empathetic, and client-centered approach
Preferred Qualifications
- Experience with electronic health records (EHR) or electronic records management systems
- Proficiency in Microsoft Office Suite, Outlook, and Internet navigation
- Certified Nursing Assistant (CNA) certification
