Health Coach

Responsibilities

• Provide one-on-one coaching sessions to help patients implement realistic, culturally appropriate lifestyle changes
• Conduct sessions in patient homes, clinics, or community settings
• Reach out to assigned patients via phone or in person
• Work with a supervising Nurse Practitioner on patient case reviews
• Support patients by teaching and motivating them to reach health goals
• Assist patients with scheduling and attending primary care visits
• Document patient visits and interactions in accordance with regulatory guidelines
• Identify and connect patients with community resources like food banks
• Communicate health complications and risks to the care team

Requirements

• 2+ years of experience in customer service, community organizations, teaching, or a related field
• High School diploma or GED
• Valid driver’s license, car insurance, and access to reliable transportation
• Ability to travel within the community for home visits
• Exceptional interpersonal and organizational skills
• Proficiency with communication and documentation technology
• Ability to lift and carry materials and supplies
• Ability to stand, walk, and move for extended periods

Preferred Qualifications

• Residence or prior long-time residence in the Cleveland community
• Bilingual skills (English/Spanish or other dominant local languages)
• Demonstrated personal commitment to health and wellness
