Group Facilitator

Responsibilities

• Facilitate person-centered TBS groups promoting emotional regulation, coping skills, and social connection
• Adapt group content for older adults with diverse cognitive and physical needs
• Build trusting relationships with residents in a safe, inclusive environment
• Collaborate with nursing home staff and clinical teams for coordinated care
• Complete documentation including group notes, biopsychosocial assessments, and Medicaid-compliant treatment planning
• Participate in clinical supervision and team meetings

Requirements

• Current Ohio license (LSW, LISW, LPC, or LPCC)
• Experience facilitating behavioral health groups
• Reliable transportation and valid driver's license
• Ability to complete Ohio Medicaid provider requirements and background checks

Preferred Qualifications

• Experience with older adults or long-term care settings
• Familiarity with EHR systems
• Training in Trauma-Informed Care
