Field Based Clinical Care Coordinator

Responsibilities

• Serve as the primary care manager and service coordinator for high-risk members with comorbid behavioral health needs
• Engage members face-to-face and telephonically to conduct comprehensive needs assessments covering medical, behavioral, functional, and socioeconomic (SDoH) factors
• Develop and implement individualized, person-centered care plans with specific goals and interventions
• Partner with internal care teams, healthcare providers, and community resources to execute care plans
• Provide essential referrals and linkages to community-based providers and internal specialized services

Requirements

• Master's Degree in Social Work OR a Bachelor's Degree in Social Work, Psychology, Special Education, Counseling, or a relevant degree with 1+ years of case management experience
• 2+ years of experience in long-term support services or working with older adults
• 1+ years of experience working with local behavioral health providers and community organizations addressing SDoH
• 1+ years of experience using MS Office (Word, Excel, and Outlook)
• Ability to travel up to 75% of the time within the assigned territory
• Must reside in Indiana and possess a valid US driver's license with reliable transportation

Preferred Qualifications

• Training or certification in Motivational Interviewing, Stages of Change, Trauma-Informed Care, or Person-Centered Care
• Experience working within a team-based care model
• Experience serving individuals with co-occurring mental health and substance use disorders
• Background in Managed Care
• Bilingual proficiency in Spanish or other languages specific to local market populations
