About the role
Responsibilities
- Serve as the primary care manager and service coordinator for high-risk members with comorbid behavioral health needs
- Engage members face-to-face and telephonically to conduct comprehensive needs assessments covering medical, behavioral, functional, and socioeconomic (SDoH) factors
- Develop and implement individualized, person-centered care plans with specific goals and interventions
- Partner with internal care teams, healthcare providers, and community resources to execute care plans
- Provide essential referrals and linkages to community-based providers and internal specialized services
Requirements
- Master's Degree in Social Work OR a Bachelor's Degree in Social Work, Psychology, Special Education, Counseling, or a relevant degree with 1+ years of case management experience
- 2+ years of experience in long-term support services or working with older adults
- 1+ years of experience working with local behavioral health providers and community organizations addressing SDoH
- 1+ years of experience using MS Office (Word, Excel, and Outlook)
- Ability to travel up to 75% of the time within the assigned territory
- Must reside in Indiana and possess a valid US driver's license with reliable transportation
Preferred Qualifications
- Training or certification in Motivational Interviewing, Stages of Change, Trauma-Informed Care, or Person-Centered Care
- Experience working within a team-based care model
- Experience serving individuals with co-occurring mental health and substance use disorders
- Background in Managed Care
- Bilingual proficiency in Spanish or other languages specific to local market populations
