Family Strengthening and Support Specialist

Responsibilities

• Engage families in person-centered planning to clarify short and long-term goals specific to individual needs
• Provide connections to community resources and educational opportunities for individuals and families
• Collaborate with Local Education Agencies and community agencies to support service delivery
• Provide ongoing support and advocacy for individuals over the age of 22 who do not receive waiver-funded services
• Maintain accurate records in the AWARDS database, including Basic Service Agreements and Person Centered Planning documents
• Provide caseload coverage for the Family Support Team as needed

Requirements

• Associate's Degree in Human Services or a related field (Bachelor's Degree preferred)
• Two years of experience in human services, developmental services, special education, or a related field
• Excellent verbal and written communication skills
• Strong organizational skills and the ability to manage multiple tasks independently
• Ability to maintain strict confidentiality and work a flexible schedule
