About the role

Responsibilities

  • Provide client-centred primary care to individuals and families, recognizing social determinants of health
  • Perform assessment, screening, diagnosis, and comprehensive treatment within full scope of practice
  • Address biopsychosocial needs, including mental health and concurrent disorders
  • Participate in an interdisciplinary model of care with physicians, nurse practitioners, and therapists
  • Utilize Electronic Medical Record (EMR) systems for accurate documentation
  • Provide after-hours on-call duties on a rotating basis
  • Participate in clinical supervision and training for medical students
  • Contribute to quality improvement plans and community outreach activities

Requirements

  • Medical Degree and postgraduate Family Medicine training
  • Certification in the College of Family Physicians (CCFP)
  • Registration with the College of Physicians and Surgeons of Ontario (CPSO) in good standing
  • Minimum of two years of experience as a primary care Physician with complex needs
  • Knowledge of anti-racist, decolonizing, and anti-oppressive practices
  • Evidence of malpractice insurance through the CMPA
  • Valid Ontario Driver's license
  • Full vaccination against COVID-19

Preferred Qualifications

  • Experience working in a Community Health Centre (CHC) or community practice setting
  • Experience working with marginalized communities and individuals facing barriers to healthcare
  • Lived experience and community connection to African, Caribbean, and Black (ACB) communities
