About the Company

Sabathani Community Center (SCC) is one of Minnesota’s oldest African American-founded nonprofits & serves a multi-cultural community of more than 50,000 community members in South Minneapolis. Founded in 1966, SCC addresses social and economic disparities in youth programming, housing, health, and education.

Responsibilities

• Carry a caseload of up to 25 families, conducting regular home visits to provide parenting education and child development guidance
• Connect families to community resources including healthcare, housing, food, and mental health services
• Conduct outreach to recruit eligible families through community organizations and schools
• Plan and facilitate monthly family workshops at the Sabathani Community Center
• Maintain accurate case management records, visit notes, and TANF eligibility documentation
• Support family resilience using the Roots: The Black Family Foundation framework

Requirements

• Bachelor's degree in Social Work, Human Services, Early Childhood, or a related field preferred
• 1-2 years of experience in child and family development, home visiting, or case management
• Valid MN Driver's License and access to reliable transportation
• Ability to pass a criminal background check
• Proficiency in Microsoft Outlook, Word, and Excel

Preferred Qualifications

• Experience with evidence-based parenting or family support practices
• Experience working with families facing housing instability or poverty
• Lived experience and cultural connection to the community served
